Limited Business Name
Business address you want the cards sending to
Date from the last 3 months

What documents can be used?

Utility bill e.g. payment for gas, electric or water. Please note: Mobile telephone bills are not acceptable
Financial statement
- bank, building society, or credit card statement showing current address. Financial information can be blanked out.
- Official letter from a bank, building society or credit card company e.g., overdraft confirmation
Letter from HMRC
Local authority tax bill valid for the current year

Please note: the proof of addresses must be from different sources. For example, we will accept a utility bill and a driving licence but not two bank statements from the same bank.
