Proof of Address and ID

For all new applications require two proofs of address and one proof of identification to complete your application. The information required for sole traders and Limited/PLCs does differ so please ensure you check the correct section.

Sole traders

PROOF OF IDENTITY

✔️ Passport OR driving licence
👍 UK Driver's licences can be used for both proof of ID and proof of address.
Please Note: If the address does not match the address where the cards are to be sent we will require a separate proof of address.
❌ Provisional driving licences are not permitted


PROOF OF ADDRESS

✔️Must show the address you are trading from - this could be a home or business premises.
✔️Must show your name and,
✔️Must date from within the last 3 months
✔️What documents can be used? Utility bills, Bank statements, letters from HMRC, Driver’s Licence
❌What documents can NOT be used? Mobile telephone bills and credit card bills are NOT acceptable

Limited and PLCs

PROOF OF ADDRESS

✔️What does the document need to show? Limited business name, Business address you want the cards sending to and MUST date from within the last 3 months
✔️What documents can be used? Utility bills, Bank statements, letters from HMRC
❌What documents can NOT be used? Mobile telephone bills and credit card bills are NOT acceptable

DIRECTOR AUTHORISATION

If you are not a named Director of the company, we will require Director Authorisation to complete your application

Information you will need for your application

Proof of address and identification

  • If you are a sole trader we will need your previous address if your current time at address is less than 3 years
  • If you require your cards to be sent to a trading address we will require proof of address for this location.  Please see the link above for accepted information
  • All applicants will be required to provide one proof of identification - Please see above for more details


Partnerships

  • Name and date of birth will be required for all partners

Director Authorisation

  • If you are not a named Director of the company, we will require Director Authorisation to complete your application (email is acceptable)


Bank details to set up your Direct Debit

  • We do not accept payment from pre-paid bank accounts
  • Partnerships with joint approval will both need to complete a Direct Debit mandate form which will be supplied by your sales contact


Card details

  • Cards can be embossed with any details such as driver name, Registration plate or simply card 1,2,3
